Definition
The model's wrong outputs, considered individually rather than as an aggregate error rate. Papers using the word are usually doing error analysis — sorting failures into kinds, showing that a headline accuracy hides a structured pattern of breakdowns. A newer line treats mistakes as training material: deliberately feed flawed reasoning together with its correction, so the model learns to notice and repair its own errors mid-generation.
